New Patients: What to expect on your first visit

 

Please arrive 15 minutes early to allow time to fill out new patient paperwork. Items to bring with you:

  • Insurance Card
  • (If applicable) L&I claim form or MVA claim info
  • Driver’s License or other government photo ID card
  • Physician’s referral to physical therapy
  • Copay if applicable

 

Wear comfortable clothing that allows free range of motion and in which the physical therapist can assess the injury site / ailment.

 

During your first session, the physical therapist will spend time with you to learn about your condition, limitations, and related health history. The provider will then take specific measurements of the injury / ailment that is causing your problem and put together a treatment plan for you. In addition, you may also receive a Home Exercise Program.

 

Physical therapy treatment plan of care / rehab treatment plan consists of multiple visits to the clinic typically twice a week for several weeks. Your specific program will depend on many different variables including your pain level and current level of mobility / range of motion.

 

Our front office staff does check eligibility and benefits as a courtesy, but we recommend that you also verify your outpatient physical therapy benefits with your insurance plan. Deductibles, co-insurance, and copays are all determined by your insurance company.
